Edward Archer (1717–1789) devoted his career to improving the treatment of smallpox and the advancement of inoculation for preventing smallpox. In the portrait Archer gestures towards a building in the distance which is probably the Smallpox Hospital in St Pancras, London, where he worked as physician. The hospital was moved to Highgate in 1860 to make way for the railway station at St Pancras.

Archer was described as a humane, judicious and learned physician. In addition to medicine he studied literature and built a valuable library. He had inherited a small fortune and did not need to profit from his work, so he did not spend much time on private practice.
